If your hospital, agency or affiliation doesn't provide an eligible e-mail, you may use a document to verify your Medical affiliation.

Acceptable documents include:

• Medical License or Board Certification
• Pay Stub from Hospital
• BLS Card / BLS Certificiation (note: CPR/AED alone is not sufficient. It must say BLS, which is specifically for healthcare workers.)
• Signed letter of status from your healthcare employer
